Care Plus Project Officer  

Location: Lincolnshire 
Salary:    £23,313 pro-rata per annum

Hours: 15 hours per week 

Contract: Permanent 

We are looking for a dedicated and flexible individual to run our Care Plus Project in the Lincolnshire area that provides a range of services to adults, children and families that have a sensory loss and associated disabilities to improve the quality of life of service users.

As part of the role you will manage a team of care support workers to ensure the smooth day to day running of Care Plus service in Lincolnshire. As part of the role, you will be:

• Providing advice and guidance to support workers on completion of tasks identified by service users support plans and with their on-going development and maintenance of independent living within the immediate (and where appropriate) wider community
• Ensuring service user’s case notes are read, regularly reviewed, case notes are accurately updated, and files are up to date within the time scale specified and using the required processes that are in place.
• Carry out regular supervisions with support workers

To be successful for this post you will have:

• Experience in working with individuals who have sensory loss and other disabilities.
• The ability to identify changing needs and report accordingly
• Excellent interpersonal and team management skills
• Flexibility to work

This role covers a large geographical area therefore the successful candidate will need to have a vehicle or access to a vehicle as well as business insurance.
